What is the NEO QI Hub Produce Path Program?
The program provides eligible patients with $100 in coupons that can be used to buy fruits and vegetables at participating farmers markets.
Who is Eligible?
- Patients of this practice who are 18 and order and
- Patients who have Medicaid or Medicare insurance (without supplemental private insurance) or who are uninsured
How Does it Work?
- Come to an Info Booth (more information below) at your doctor's office and sign up
- Use the coupons to shop at participating farmers markets through June 30, 2025
What Do I Get When I Sign Up?
- $100 in coupons to buy fruits and vegetables at participating farmers markets
- A reusable shopping bag
